Job Title: (CNA, PCA, HHA) Companion Caregiver

Pay Rate: $30 per visit
Schedule:  Must be available Mondays through Thursdays from 7 PM until 7:30PM

Job Description: 
Collaborative Home Care is looking for a compassionate and reliable caregiver to provide support and companionship for an elderly woman with cognitive impairment (dementia). The ideal candidate will be patient, attentive, and experienced in senior care. Responsibilities include assisting with evening routines and helping the client get ready for bed, along with providing supervision and support throughout the shift.

Additional Opportunities:
Caregivers who demonstrate reliability, professionalism, and excellent service may be eligible for placement on additional cases within our company.

The Care Plan includes:

a) Assist client with toileting
b) Prepare client for bedtime
c) Provide supervision and support for cognitive impairment
d) Accompany and walk with the client 

 Qualifications: 

• A love for the senior community
• A compassionate, upbeat attitude and patient demeanor
• Strong commitment to reliability and ethical behavior
• Ability to maintain confidentiality and professionalism
• Ability to communicate effectively in both written and verbal forms
• Dedication to professional development
• A minimum of 1-year experience in caregiving for seniors
• Ability to lift 30–40 pounds
• Possess a valid Driver's License with a clean driving record, Auto Insurance and a reliable and well-maintained car
• Possess a State ID and a valid second form of ID, such as a Social Security Card, Passport, or Birth Certificate
• Eligible to work in the United States for any employer
• Pass a background check and drug testing per company standards

Responsibilities and Duties:

Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders)
Companion care (e.g. conversation, hobbies and activities, shopping).
Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
