Job Title: (CNA, PCA, HHA) Companion Caregiver

Location: Stamford/Greenwich, CT Area
Pay Rate: $21 per Hour
Schedule:  Must be available Mondays through Fridays from 9 AM until 6 PM

Job Description: 
Collaborative Home Care is looking for a compassionate and reliable caregiver with a positive, can-do attitude. The ideal candidate will be enthusiastic, proactive, and committed to supporting an elderly woman. Responsibilities include companionship, assistance with meal preparation, light housekeeping, and providing transportation for social events, medical appointments, and errands. A valid driver’s license and access to a reliable vehicle are required, with mileage reimbursement provided for personal vehicle use.

The Care Plan includes:

a) Engage in activity (companionship)
b) Light housekeeping and meal preparation
c) Occasional driving trips to run errands, social activities and doctor appointments
d) Medication reminders
e) Miles driven in your car will be reimbursed

 Qualifications: 

• A love for the senior community
• A compassionate, upbeat attitude and patient demeanor
• Strong commitment to reliability and ethical behavior
• Ability to maintain confidentiality and professionalism
• Ability to communicate effectively in both written and verbal forms
• Dedication to professional development
• A minimum of 1 year of experience in caregiving for seniors
• Ability to lift 30–50 pounds
• Possess a valid Driver's License with a clean driving record, auto insurance, and a reliable and well-maintained car
• Possess a State ID and a valid second form of ID, such as a Social Security Card, Passport, or Birth Certificate
• Eligible to work in the United States for any employer
• Pass a background check and drug testing per company standards

Responsibilities and Duties:

Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders)
Companion care (e.g. conversation, hobbies and activities, and shopping).
Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
