Pay Rate: $22.00 per Hour
Schedule: Availability required: Mondays from 10:15 a.m. to 6:30 p.m. and Wednesdays from 9:00 a.m. to 4:00 p.m.


Job Description:
Collaborative Home Care is seeking a compassionate and reliable part-time companion caregiver to provide assistance and support to our client on Mondays & Wednesdays. The client needs help with medication reminders, meal preparation, light housekeeping, driving to doctor appointments, and social activities. An important part of this position is driving the client to therapy appointments. Some of those appointments require extensive driving (e.g., New Haven). The client has a car you can drive. You must have a clean driving record. 

The Care Plan includes:
a) Companionship
b) Meal preparation
c) Light housekeeping 
d) Driving trips to doctor appointments and social activities
e) Medication reminders

Qualifications: 
  • A love for the senior community
  • A compassionate and upbeat attitude
  • Strong commitment to reliability and ethical behavior
  • Ability to communicate effectively in both written and verbal forms
  • Dedication to professional development
  • A minimum of 1-year experience in caregiving for seniors
  • Ability to lift 30-40 pounds
  • Possess a valid driver's license, access to reliable transportation, valid auto insurance and a good driving record
  • Possess a State ID, and a valid second form of ID, such as a Social Security Card, Passport, or Birth Certificate
  • Eligible to work in the United States for any employer
  • Pass a background check and drug testing per company standards

Responsibilities and Duties: 

  • Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
  • Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders)
  • Companion care (e.g., conversation, hobbies and activities, and shopping).
  • Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
