Job Title: CNA, PCA, HHA Dementia Experienced Caregivers
Location: New Haven, CT area
Schedule: Must be available to work on Monday through Thursday from 9am to 3pm.
Pay Rate: $21 per hour

Job Description:
Collaborative Home Care is currently seeking a compassionate and reliable caregiver who embodies a CAN-DO attitude to help an elderly woman living with dementia. The ideal candidate will possess a positive, proactive attitude along with extensive dementia experience and compassion to assist elderly clients. The client needs supervision for safety, help with personal care, mobility assistance, meal preparation, and light housekeeping. The client needs to be driven to doctor's appointments and errands so a valid driver’s license and reliable transportation are required. Mileage reimbursement will be provided for the use of your vehicle.

The Care Plan includes:
a) Extensive Dementia experience 
b) Supervision for client's safety
c) Helping client with personal care such as showering and dressing, assist in using the toilet
d) Assistance with mobility support
e) Light housekeeping and meal preparation
f) Occasional driving trips to doctor appointments and errands
g) Medication reminders
h) Mileage reimbursement

Qualifications:
 

• A love for the senior community
• Has an extensive dementia experience 
• A compassionate and upbeat attitude 
• Strong commitment to reliability and dependability
• Self-directed, with the ability to work independently
• Ability to communicate effectively in both written and verbal forms
• Dedication to professional development
• A minimum of 1-year experience in caregiving for seniors
• Ability to lift 30-50 pounds
• Possess a current valid Driver's License with a clean driving record, Auto Insurance and a reliable and well-maintained car
• Possess a current State ID and a valid second form of ID, such as a Social Security Card, Passport or Birth Certificate
• Eligible to work in the United States for any employer
• Pass a background check and drug testing per company standard

Responsibilities and Duties:

• Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
• Providing care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders) 
• Companion care (e.g. conversation, hobbies and activities, shopping, driving to appointments).
• Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
• Establish and maintain effective communication and a professional relationship with clients, family members, and co-workers
• Perform other duties as assigned on the personal care service plan.
